Ambiguous responsibility

For example, someone sends a request to an organization's/web site's e-mail address, an e-mail account which is checked by multiple people. Bzzt! Problem. Division of responsibility is the most request cause of ambiguous problem. Who is responsible to handle that request? You? Me? Unless the responsibilities of each person are clearly defined, there is likely to be ambiguity.

And the result of that is reluctance to take the task. So the request gets neglected. "Ah, somebody else will surely take care of it." Or "That's not my responsibility, that's someone else's ... I think."
